Routine Laboratory Services
Patients will no longer need to schedule lab appointments. A physician order is required and patients may call to pre-register at 419-483-4040, then dialing 0 for the operator.
Please remember to wear your own cloth face coverings upon arrival to our hospital, outpatient clinics and physicians’ offices and to keep the covering on while traveling through our hospital and medical offices.

Family Health Services - Clyde Laboratory
Family Health Services has reinstated laboratory services at their Clyde location. Please note: COVID-19 testing will not be
done at the Clyde location.

Drive-Thru COVID-19 Testing
Beginning Monday, March 8, The Bellevue Hospital will transition to drive-up COVID-19 testing at 1400 W. Main St., Ste. E. This new site is located in the medical offices behind the hospital.
COVID-19 drive-thru testing hours:
Mondays - Fridays, 9:00 a.m. – 5:30 p.m.
Please be sure to have a mask or face covering on before pulling up for testing. Patients should remain in their vehicle until a member of our staff can assist. A physician order is required and insurance information will be collected at the time of testing. Please note, no pre-registration is needed for COVID-19 drive-thru testing. Registration will be done at the car prior to testing.
Outpatient COVID-19 collection will be completed at the COVID-19 drive-thru testing site with the exception of those patients who need additional bloodwork or are getting tested as part of pre-admission testing for surgeries. All other laboratory services will be offered through the Main Laboratory at the hospital, 1400 W. Main St., and Family Health Services outpatient laboratory services, 402 W. McPherson Highway, Clyde.
The map below may be used to guide patients to the collection site (noted using the yellow arrows). Once the testing is complete, patients should exit the hospital campus via the County Road 302 exit (noted using the red arrows).
Accessing Your Test Results
The Bellevue Hospital knows it is important to know your results. Here are some options for accessing your results:
1. If you have access to TBH’s patient portal, FollowMyHealth, you may find your test results this way. If you are already
signed up for the portal, log in at: https://bellevuehospital.followmyhealth.com.
If you are not signed up for the patient portal, follow the directions in the brochure below to set up your account.

2. Wait for your physician to contact you with the results. Turnaround times vary depending on the test.
Please note: Turnaround times for COVID-19 test results may take one to two days.
3. Although Health Information Management (Medical Records) is currenly closed to in-person visits, you can call 419.483.4040, Ext. 4212 or email email@example.com for release of your medical records.
