What is the Health eChart patient portal?
The Health eChart patient portal provides you with confidential and secure access to your health information. From the convenience of your home or on your mobile device, you can access test results, your medical history, upcoming appointments, and medications and allergies.

 

Where does my health information on the Health eChart patient portal come from?
All of the information contained in your profile on the Health eChart patient portal is information from your electronic health record from The Bellevue Hospital.

 

Is there a fee to use the Health eChart patient portal?
No, use of the Health eChart patient portal is provided free of charge to any patients of The Bellevue Hospital.

 

How can I get registered to use the Health eChart patient portal?
There are several ways for you to register for the Health eChart patient portal:

  • Upon patient registration, you will be asked if you would like to register for the patient portal. Please supply your correct email address during the registration process. A secure invitation will be sent to you via email. Your email invitation to the Health eChart will enable you to log in and create your own username and password.
  • Once you have visited TBH and are in the new Electronic Health Record System, Meditech Expanse, you may self-register directly online at thebellevuehospitalportal.meditech.cloud

Will my health information in TBH’s previous FollowMyHealth patient portal be transferred to the new Health eChart patient portal?
The transition to the new Health eChart will not include your previous medical data. The information that is available in the Health eChart patient portal will only include data and entries after June 1, 2023. However, you will still have access to your health information stored in your FollowMyHealth account. To access your previous health information in FollowMyHealth, visit bellevehospital.followmyhealth.com.

 

Can my family access my health information?
In addition to having access to your own health information, the Health eChart patient portal also allows you to grant others, as you wish, to have access to your health information. Individuals at least 18 years old can be given proxy access to your health records with an authorized consent to help manage your care. If the patient is a minor, authorization by the minor’s parent or legal guardian is required to grant proxy access. In order to comply with U.S. and Ohio laws and regulations concerning the privacy of certain medical/mental health records of minor patients, minors age 13 to 17 must also provide authorization for access. The Bellevue Hospital does not provide Health eChart website access to the electronic health record of minor patients from the ages of 13 to 17 without consent. Who do I contact for possible errors in my record?
Your information on the Health eChart patient portal comes directly from your electronic health record. If you find errors, please contact our Health Information Management department at records@bellevuehospital.com, or by calling 419.483.4040, Ext. 4262 to correct any inaccurate information.

 

What if I have technical problems with the Health eChart patient portal?
If you have technical problems with the Health eChart patient portal, please contact us at 419.484.5475.

 

Can I access my child’s health records from my account?
Yes, but only for children up to 13 years of age. In order to comply with U.S. and Ohio laws and regulations concerning the privacy of certain medical/mental health records of minor patients, The Bellevue Hospital does not provide Health eChart website access to the electronic health record of minor patients from the ages of 13 to 17 without the minor’s signed consent. Can I request an appointment through the Health eChart patient portal?
Not at this time. However, The Bellevue Hospital hopes to offer this service in the future.

 

Can I access the Health eChart patient portal through my smartphone?
Yes. The Health eChart patient portal can be accessed using an Android or iPhone through the "MEDITECH MHealth" app. Once you have installed the app, you will have to choose the portal for The Bellevue Hospital. If you need assistance, please contact us at 419.484.5475.
