If you have been pre-registered, please report to the department performing your test or procedure unless otherwise instructed.
Please have the following information:
- Physician's order
- Current insurance card
- Driver's license, or other form of identification
If we do not have a copy of your current insurance cards and / or driver's license, we will ask you for them upon your arrival.
