The Bellevue Hospital offers the choice of two medical plans, plus dental and vision plans built to meet the needs of you and your family.
The biweekly employee contributions for both plans are withheld from your paycheck on a pre-tax basis through our Section 125 Cafeteria Plan. Premiums withheld on a pre-tax basis lower your taxable income.
Preferred providers under both plans include our own facility and many local physicians (Tier 1, Bellevue Health Network). For care outside of our immediate area, we utilize Medical Mutual of Ohio.
Traditional PPO Health Plan
The Traditional Medical Plan features low physician office visit co-payments. Many services are paid at a 100% benefit after meeting a modest deductible when using a local provider.
Qualified High Deductible Health Plan
The Qualified High Deductible Health Plan features low biweekly premiums and a $1,700 single or $3,400 family deductible. Preventative services provided by a Tier 1 provider are paid at a 100% with the deductible waived.
Both plans include a pharmacy benefit through CVS Caremark. Prescriptions can be filled at a retail pharmacy or through convenient mail order.
Health Savings Accounts and Flexible Spending Accounts
Depending on your eligibility, you may participate in either a flexible spending account or health savings account. Both plans allow you to use pre-tax dollars to pay for qualified medical expenses.
Condition Management Program
When you enroll in the TBH medical plan, you may participate in a condition management program which focuses on seven chronic conditions such as high blood pressure, high cholesterol, diabetes, etc. Those who meet the participation criteria may be eligible to receive prescriptions related to their condition at no cost.
BeWELL For Life Incentive Plan
The BeWELL For Life Incentive Plan allows you to earn points by having preventative screenings and participating in BeWELL programs. With the points earned, you qualify for lower employee medical plan contributions and TBH contributions to either a flexible spending account or health savings account.
The dental plan is administered through Delta Dental. Preventative services are paid at 100% with no deductible. The plan includes orthodontia benefits for dependents up to age 26.
The TBH vision plan is administered through VSP. The plan features annual eye exams, frames, lenses and contact lenses with a co-payment.
