TBH Medication Management Clinic Temporary Relocation

Starting July 23, 2018, The Bellevue Hospital’s (TBH) Medication Management Clinic will be temporarily re-located from Medical Building #1 behind the hospital, to the Cardiopulmonary Department inside the hospital, located at 1400 W. Main Street.

Due to current construction on the hospital campus, individuals should use the East Cardiopulmonary Entrance (labeled entrance “E”) for the Medication Management Clinic.

The Medication Management Clinic is geared toward patients taking anticoagulant medication in the short- and long-term. Some examples of these medications are Coumadin (warfarin), Xarelto, Pradaxa, Eliquis, and Lovenox. Anticoagulants are taken to prevent and control the growth of blood clots. Medication Management is important since anticoagulation medications can be dangerous if not monitored and managed, especially in elderly individuals. The Medication Management Clinic offers personalized treatment plans that are monitored by a certified anticoagulant pharmacist.

TBH’s Medication Management Clinic is currently open on Tuesdays, Wednesdays, and Thursdays from 8 a.m. until 4:30 p.m., by appointment. Patients will need a referral from their family physician and should also check with their primary insurance carrier to see if visits to the clinic are covered.

For additional information, TBH’s Medication Management Clinic staff can be reached at 419.484.5970.
