Insurance Information

As a service to you, The Bellevue Hospital Patient Financial Services staff will bill your insurance provider.  Balances remaining after insurance will be billed to you on a patient statement.  You may also be billed if your insurance company denies payment or fails to respond.

Please remember that you are ultimately responsible for payment of the account.

However, patients should also contact their member services department to inquire for possible restrictions or network levels for tiered benefit plans. 

 

*Note: If you were involved in an auto accident please complete the accident information questionnaire during registration.  We also ask you provide us with auto insurance information and medical insurance to assist you with your claim processing.  If we do not receive the information the account will become a self pay bill.
