The Bellevue Hospital would like your visit to be as smooth and quick as possible.  If you would like to pre-register for your visit, please call us a day or two ahead of time and we will be glad to pre-register you.

Some Things to Remember:

• When calling to pre-register or registering in person, please have the following information:  Your current insurance card and driver's license (or other form of picture id).

• If you have been pre-registered, please stop at the front desk to sign the consent form upon arrival.

• If we do not have a copy of your current insurance cards and/or driver's licensed, we will ask you for them  upon your arrival.
