The Health Information Management
(HIM) Department is responsible for maintaining
medical records. We perform assembly, analysis,
transcription, coding, scanning, and release of
information.

- 5:00 p.m. We are closed on weekends and
holidays.

Obtaining Copies of your Medical Record

Cost associated with release of information

Each year the state of Ohio
releases information on what hospitals can
charge to provide copies of medical records.
It is best to contact our department and
we will be happy to provide you with this
information.

Birth Certificates
-
The Bellevue Hospital does
not have copies of birth certificates at the
hospital. If you would like a copy of your
birth certificate, go to the Ohio Department
of Health’ web site at
http://www.odh.ohio.gov/vitalstatistics/vitalstats.aspx
to review information on how to obtain a
birth certificate or other vital statistics.
Anyone born at The Bellevue Hospital may also obtain
a birth certificate by contacting the Huron County
Health Department.
