Birth Certificates

• The Bellevue Hospital does not have copies of birth certificates at the hospital. If you would like a copy of your birth certificate, go to the Ohio Department of Health’ web site at http://www.odh.ohio.gov/vitalstatistics/vitalstats.aspx to review information on how to obtain a birth certificate or other vital statistics.  

• Anyone born at The Bellevue Hospital may also obtain a birth certificate  by contacting the Huron County Health Department.
